OUR REFERRAL PROCESS
When a patient is referred, our process begins with a thorough review of the referral information to ensure all necessary details are understood. We then contact the patient promptly to schedule an initial consultation, during which we assess their medical history and current condition. Relevant diagnostic tests or evaluations are arranged as needed to form a comprehensive understanding of the patient's needs. Throughout the process, we maintain clear communication with both the patient and the referring provider to coordinate care effectively and ensure a seamless transition.
1. Submit the Referral
Referring providers or community partners complete the secure online referral form with basic patient and contact information.
2. Intake Review & Pre-Visit Setup
Our intake team reviews the referral within 1–2 business days and begins the onboarding process. Patients (or guardians) receive digital consent forms, a brief clinical questionnaire, and a demographic and insurance form to complete before their first appointment. This step helps us verify coverage, confirm identity, and prepare the provider with essential background information.
3. Patient Contact
Once intake is complete, our care coordinator reaches out directly to schedule the initial 60-minute telepsychiatric evaluation and answer any questions about the process or technology.
4. Initial Evaluation
During the first 60-minute session, a licensed clinician or certified professional performs a comprehensive assessment covering symptoms, history, lifestyle, and treatment goals. Together, we create a personalized care plan aligned with the patient’s needs and values.
5. Treatment Planning & Follow-Up
The treatment plan may include therapy, medication management (non-controlled substances), and holistic or faith-based support. If medication is started, we schedule a 2-week 30-minute follow-up to assess response and adjust the plan as needed, followed by monthly visits once stable..
6. Ongoing Communication & Collaboration
With patient consent, we maintain communication with referring providers to share updates, coordinate care, and ensure continuity throughout treatment.
